Distal Hypospadias Surgery

The opening is at or just below the head of the penis, and there is no significant curvature. This is the most common and best-outcome form of hypospadias; repair is completed in a single operation, and the child usually goes home the same day or the next.

01

The three goals of the operation

All three are done in the same session. The channel is missing only the last few centimeters of its proper length, so rather than building a tube from scratch, the existing groove is simply closed over.

  • Moving the opening to the tip of the head.
  • Making the urine stream straight and single-directional.
  • Closing the head so it looks natural.
02

Techniques used

  • TIP (Snodgrass): the groove is deepened along the midline and tubularized. This is the first choice in most distal cases and gives a natural-looking, slit-shaped opening.
  • MAGPI / GAP: a more limited correction used when the opening is nearly at the tip and the groove is very good.
  • Mathieu (flip-flap): closing the channel with a flap raised from the skin just below the opening; comes up when the groove is narrow.
  • A protective interposition layer (dartos flap): living tissue is laid between the new channel and the skin. It is one of the most important technical details for preventing a fistula.

Surgery and afterward

  • Surgery: 1–2 hours under general anesthesia; day surgery or a single overnight stay at most centers.
  • Pain: a penile-block or caudal block given during surgery keeps the first hours comfortable; simple pain relievers are enough afterward.
  • Catheter: a thin catheter stays in for 5–7 days and drains into the diaper; removing it is painless.
  • First 2 weeks: swelling and bruising are normal. Diapers are changed often, the area is kept dry, and bathing starts with your doctor's permission.

When is circumcision done?

At the end of the operation, while the skin is being tailored. If hypospadias is suspected, circumcision should not be done beforehand: the foreskin is a backup tissue that may be needed for the repair.

Frequently asked questions
What is the best age for surgery?
Generally 6–18 months is preferred. Tissue healing is good during this period, the child will not remember the experience, and it falls before toilet training. Surgery is also done outside this range; only the preparation and expectations change.
Is a second operation needed?
In suitable distal cases, there is no planned second stage. Additional treatment only comes up if a complication develops, such as a fistula or stricture.
When does the child return to normal life?
In the days after the catheter comes out. Follow your doctor's timeline for daycare and play; a full return usually happens within 2–3 weeks.

With the hypospadias assessment that fits your situation would you like to reach the doctor?

There are two ways. If you'd like the doctor to already know your situation before replying, start with the short assessment; if your question is brief, write to us directly. Both reach the same team.

1 Let me assess my situation firstRecommended The Roadmap asks a few questions in about 2 minutes, collects your answers with an anatomical diagram, and produces a ready-made summary. That summary is added to your WhatsApp message — the doctor replies already knowing your situation. Start the Roadmap
2 I'll write directly Fill in the form; your message opens ready-made in WhatsApp (you can edit it before sending), or you can send it directly by email. This page's topic is added to the message automatically.

Editor's note: Age and a short sentence are enough; you don't need to use medical terms. Please don't send photos — the doctor will request them through a secure channel if needed. What you write is only sent when you submit it; this page does not store anything.
