The short answer

While curvature is being corrected, the unhealthy tissue on the underside of the penis is removed, leaving a gap. Tissue needs to be added to this gap. But added tissue can't be shaped into a tube until it establishes its own blood supply — because a channel built from tissue without a blood supply contracts and ends in stricture.

  • That's why the foundation is prepared in one session, time is allowed for it to mature, and the channel is built in the second session.

Why not force it into a single session?

It can be done, but the result isn't reliable. When a channel built on tense, poorly perfused tissue fails, three or four corrective surgeries can follow. A planned two-stage approach is a predictable path.

  • In other words, two stages isn't a choice that increases the number of surgeries — it's the choice that reduces the total number of surgeries.

When does this come up?

  • When the opening is proximal (the base, the penoscrotal junction, the scrotum, the perineum).
  • When curvature still exceeds 30 degrees despite releasing the skin and clearing the band.
  • When the urethral plate is narrow, thin, or scarred and can't be preserved.
  • When there has been previous surgery and tissue is inadequate.

Does two stages mean failure?
No. Staged repair is a path planned from the start; it is not a correction done because of a complication. Knowing this distinction makes the process much easier.
When will we know the decision?
In some cases it can be predicted at the examination, but the final decision is made in the operating room once the plate is seen and the curvature is measured. That's why both possibilities are discussed during the consent process.
