Urethral Stricture After Hypospadias Surgery in Children

If the stream is weak but the opening itself doesn't look narrow, the stricture lies further inside the channel. This picture isn't solved by procedures aimed at the opening alone; the stricture's location and length need to be identified.

How does it develop?

It can result from scar tissue along the repair line, insufficient blood supply, or shrinkage of part of the graft.

  • The findings progress quietly: the stream thins over time, urination takes longer, the bladder doesn't empty fully, and infections start to recur.

Diagnosis

  • Measuring flow rate with uroflowmetry (if the child's age allows it).
  • Measuring the urine left in the bladder after voiding, with ultrasound.
  • Imaging the location and length of the stricture with a contrast study (urethrography) — this is the basis of the plan.
  • Endoscopic evaluation if needed.

Treatment

  • Very short stricture: endoscopic incision can provide a short-term solution; strictures after hypospadias surgery tend to recur.
  • Short segment: removing the stricture and joining the ends together.
  • Long segment: widening with buccal mucosa; long-term results are more reliable.
  • Extensive damage: a staged repair.

An important caution

How many centimeters is the stricture, and why does it matter?
The choice of treatment depends directly on its length. A short stricture and a long stricture are not solved with the same method — this is why a decision cannot be made without imaging.
How do I know if my bladder has been affected?
Measuring the amount of urine left in the bladder after voiding with ultrasound is the most practical method. Recurring infection is also a sign in this direction.
