What Is Chordee? Penile Curvature in Hypospadias

Curvature is the second key determinant in hypospadias, after the position of the opening. Repairing the channel without first correcting the curvature only builds it on a penis that stays bent — that's why the order is always: curvature first, channel second.

What is chordee?

Chordee is the downward bending of the penis during erection. Turkish sources call it penile curvature, and in everyday language it's sometimes called a "crooked penis" — all describe the same finding.

  • Chordee is common in hypospadias but not present in every case. In some children whose opening is close to the tip there is no curvature at all; the further back the opening sits, the more likely curvature becomes.
  • The term penile chordee is used to specify that the curvature is in the shaft of the penis. Curvature can also occur on its own, without hypospadias — in that case the evaluation follows the same steps.

Where does the curvature come from?

There are three separate sources, and more than one can be present together in a single case:

  • Skin tightness — the skin on the underside of the penis is too short. Once the skin is released, this component disappears.
  • A fibrous band — unhealthy connective tissue beneath the channel. Once it is cleared, part of the curvature resolves as well.
  • A length mismatch between the erectile bodies — the underside of the structures that form the shaft is shorter than the top side. This is the most resistant component.

How is it measured?

The definitive measurement is made during surgery with an artificial erection test. Saline is injected to produce an erection and the angle is measured; the test is repeated after the skin is released and the band is cleared.

  • The usual decision point is around 30 degrees. Curvature below this angle does not affect function; above it, additional correction is needed.

How is it corrected?

  • Skin release (degloving) — the first and simplest step.
  • Clearing the fibrous band.
  • Dorsal plication — sutures placed on the top surface balance out the length mismatch. Simple; some shortening is expected.
  • Adding tissue to the underside (a graft) — the shorter side is lengthened. Length is preserved; the procedure is more extensive.

How can I notice curvature in a baby?
In babies, erection is seen toward early morning, during a nappy change, or while crying. Look from the side at that moment and take a photograph if you can — it's genuinely useful at the examination.
Can curvature come back after surgery?
As the penis grows during puberty, a length mismatch that wasn't noticeable at a young age can become apparent. That's why hypospadias follow-up continues into adolescence, when a reassessment is done.
