Redo Hypospadias Surgery (Repeat Repair in Children)

Multiple procedures have already been performed, and the problem persists. This is the most demanding area of hypospadias surgery, and by this stage the plan is no longer built around standard techniques but around an inventory of the tissue that remains.

Why is it planned differently?

Every operation reduces two things: the amount of usable healthy tissue and the blood supply to that area. Because of this, a third procedure can't be planned with the same technique as the first. The surgeon's first task is to work out what's left to use — penile skin, foreskin, or oral mucosa.

02

Two common mistakes

  • Rushing: procedures performed before scar tissue has softened tend to break down again. At least 6 months is usually allowed to pass.
  • Addressing problems piece by piece: stricture, fistula, and curvature often feed into one another, and repairs done without mapping all of them together rarely hold up long term.
03

Preparation

  • Gathering all previous operative reports — which technique and which tissue were used.
  • Mapping the entire channel with contrast imaging and, if needed, endoscopy.
  • Taking stock of the remaining tissue.
  • Assessing the skin for lichen sclerosus.
  • Measuring voiding function and the state of the bladder.
04

Repair

The standard approach is a staged repair: in the first stage, all scar tissue is cleared, the curvature is corrected, and the base is prepared with a graft; in the second stage, the channel is built. Oral mucosa is the most reliable tissue in this group.

  • The process is spread across two, sometimes three operations, and can take more than a year in total. This isn't the process dragging on — it's what's needed for a lasting result.

Frequently asked questions
Wait at this stage, or operate right away?
A procedure performed before scar tissue has softened is likely to break down. Correct timing can matter more for a lasting result than the technique itself.
How many more operations will be needed?
Two stages are usually planned; in some cases a third operation is needed. Once the inventory of remaining tissue has been taken, your surgeon can give you a clearer picture.

With the hypospadias assessment that fits your situation would you like to reach the doctor?

There are two ways. If you'd like the doctor to already know your situation before replying, start with the short assessment; if your question is brief, write to us directly. Both reach the same team.

1 Let me assess my situation firstRecommended The Roadmap asks a few questions in about 2 minutes, collects your answers with an anatomical diagram, and produces a ready-made summary. That summary is added to your WhatsApp message — the doctor replies already knowing your situation. Start the Roadmap
2 I'll write directly Fill in the form; your message opens ready-made in WhatsApp (you can edit it before sending), or you can send it directly by email. This page's topic is added to the message automatically.

Editor's note: Age and a short sentence are enough; you don't need to use medical terms. Please don't send photos — the doctor will request them through a secure channel if needed. What you write is only sent when you submit it; this page does not store anything.
