The Urethral Plate: The Structure That Decides Single- vs. Two-Stage Repair

The goal of hypospadias repair is to reconstruct the missing part of the channel. If a sound plate is present, the surgeon can deepen this foundation and turn it into a tube instead of building one from scratch. That means less tissue needs to be moved, and the operation is shorter.

• If the plate is inadequate — narrow, thin, or covered in scar tissue — a channel can't be built on it. In that case, the plate is removed and a new foundation is prepared from other tissue instead. Because that added tissue can't be turned into a tube until it has established its own blood supply, the repair is split into two stages.

This assessment can only be made during surgery, after the skin has been released. The impression formed at examination is a guide, but the final decision is made on the table.

• Width: a narrow plate can be widened by deepening it along the midline.
• Flexibility: a stiff, tight plate causes a stricture when it's turned into a tube.
• Blood supply: a pale plate with poor circulation isn't preserved.
• Length: determines how long the channel that needs to be built will be, based on where the opening sits.

If your surgeon says, "If I can preserve the plate, I'll finish it in one operation," this is the reason for the uncertainty before surgery. That isn't indecision — it's the nature of this surgery. Both scenarios should be discussed as part of the consent process.
