Curvature seen in the flaccid state doesn't reflect the true angle. Curvature that isn't noticeable from the outside can show up in this test — this is called hidden chordee.

• The usual decision point is around 30 degrees. Curvature below this angle doesn't typically affect function; above it, additional correction is needed.

• Before the skin is opened: the initial angle is measured.
• After the skin is released: once the tightness-related component is removed, the remaining angle is seen.
• After the fibrous band is cleared: the true remaining angle is measured, and the decision about additional correction is made at this point.

This test can change the surgical plan. In a case where the opening looks distal, it can shift further back once the fibrous band is cleared, and the repair can become more extensive. This is expected and should be discussed during the consent process.
