Urethral Stricture After Hypospadias in Adults

The channel has narrowed following a repair done years earlier. This is the most common late complication of hypospadias surgery, and it usually appears during adolescence or adulthood.

Why does it appear late?

As the penis grows during adolescence, the repaired channel cannot grow at the same rate. Scar tissue contracting over the years adds to this.

  • Because symptoms progress slowly, most people assume the situation is normal: the stream thins, voiding takes longer, and straining begins. Over time the bladder no longer empties fully and infections recur.

The most critical warning

Repeated endoscopic cutting (urethrotomy) procedures give temporary relief but make the stricture larger and make the later open repair harder. This method should not be repeated for strictures that follow hypospadias surgery.

Diagnosis

  • Flow rate with uroflowmetry.
  • Ultrasound measurement of urine remaining in the bladder after voiding.
  • A contrast study (urethrography) to locate and measure the length of the stricture — the basis of the plan.
  • Endoscopic assessment.
  • Skin examination for lichen sclerosus.

Treatment

  • Short stricture: removing the narrowed segment and joining the ends.
  • Most common: widening with an oral mucosa graft; long-term success rates are high.
  • Long or extensive stricture: staged urethroplasty.
  • Endoscopic cutting: considered only for very short, first-time strictures, and should not be repeated.

How long does the catheter stay in?
Usually 2–3 weeks when a graft is used. A contrast study may be done before it is removed.
How long does follow-up last?
Flow measurement at months 3, 6, and 12, then yearly checks. Strictures can recur even years later.
